Skid-Steer Case

A newly hired construction worker was injured when a skid-steer operator backed over him. My report focused on whether there was negligence of the construction company regarding inadequate training of the construction worker and the skid-steer operator.

Lockout/Tagout Case

An employee with over a decade of lockout/tagout training at a food processing facility was injured when the employee did not lockout a conveyor belt. My investigation and report focused on whether an adequate lockout/tagout training program was in place at the food processing facility.

Rough Terrain Telehandler Forklift Case

A contract truck driver was injured in a fall from an empty hay trailer at a hay farm. A rope used to secure the hay bales became entangled on the forks/tines of a rough terrain telehandler forklift resulting in the injury. My investigation focused on whether there was negligence of the farm regarding safe operation of the rough terrain telehandler forklift.
